Improving care by delivering the Chronic Care Model for diabetes

Despite robust evidence-based guidelines for diabetes care goals, the majority of patients do not reach these goals. This is not so much a shortcoming of providers or patients, but rather reflects our healthcare delivery system. Implementation of the Chronic Care Model has been shown to improve outcomes for diabetes by providing a system for productive interactions of a prepared proactive practice team and an informed empowered patient. The Chronic Care Model is the most evidence-based approach to transform primary care, where most patients with diabetes are seen. Increased focus on healthcare professionals implementing this robust model of care across different practice settings is needed to improve diabetes outcomes. In the future, appointments for patients with diabetes will also evolve to become more patient centered.
