Lumbar spine fusion is a common procedure that involves a high cost burden and a risk of serious complications. We aimed to summarize systematic reviews of the efficacy of lumbar spine fusion in most diagnoses. No high-quality systematic reviews were found and the risk of bias of randomized controlled trials in the reviews was generally high. The evidence available does not support the benefit of spine fusion compared to non-operative alternatives for back pain associated with degeneration. The available evidence does not support the clinical benefit of spine fusion compared to non-operative treatment or non-fusion stabilization for thoracolumbar burst fractures. The benefits of spinal fusion compared to non-operative treatment for isthmic spondylolisthesis are unclear.
