 The first link in the Survival Chain is early identification of declining patient and the avoidance of cardiac arrest. When heart attack happens, less than 20 per cent of people with cardiac arrest at the hospital will live and go home. Time of cardiac arrest is the known time, just immediately before or just after a total cardiac arrest, where the state of the patient is very unstable and precautions must be taken to avoid deterioration or relapse into full cardiac arrest. Thus, all unstable patients are at risk of heart arrest. Rapid identification of reversible causes of cardiac arrest is crucial for the correct treatment of these patients and their outcome. As well as precociously identifying the conditions of peri-cardiac arrest prevents heart arrest. Ultrasound appears to be an ideal diagnostic tool. Integrating data which can be quickly obtained from examination of the heart, lung, inferior vena cava, abdomen and leg vein is often essential for diagnosis and treatment.
