Formal pain assessment is critical for initiating and evaluating pain treatment effectiveness. Subjectivity of assessment is reduced through the use of an evaluation tool. There are two forms of methods for pain assessment, sometimes self-reporting and often subjective or clinical for individuals who are unable to express themselves. Self-reporting: Uni-dimensional and multi-dimensional self-reporting tools are the most reliable pain measure as long as the pain person is heard and believed. The pain severity is the most clinically important aspect of the pain experience, according to Hjermstad et al.; hence it is the most frequently measured feature of pain using uni-dimensional instruments. Tools for evaluating pain intensity include the Visual Analog Scale (VAS), Numerical Rating Scale (NRS), Verbal Descriptor Scale (VDS), Numerical Descriptor Scale (NVDS) and Wong Baker smiley faces, Behavioral pain evaluation tools.
