Author(s): Sushil Upadhyay

A middle aged lady presented with acute onset respiratory insufficiency without any predisposing factors. Pulmonary, cardiac, neurological and infectious causes were excluded after detailed examination. Bilateral diaphragmatic palsy was considered after meticulous survey. She benefited with noninvasive bilevel positive airway pressure (BiPAP) support. High degree of clinical suspicion is stressed in diagnosing bilateral diphragmatic palsy and execellent results on treatment with non-invasive ventilation.
